HOW DO I REQUEST A REPEAT
PRESCRIPTION?
Repeat prescriptions are issued by
computer. The order form is to be
found on the right hand side of your
prescription. Please tick those
medications you need. This then
needs to be handed into the surgery
or posted with a stamped addressed
envelope.
You can send
your request via our online form -

Alternatively you
can order via
SystmOnline
Or your request can be faxed to 0113
2954228.
We cannot accept telephone requests
for repeat prescriptions due to
safety issues.
Please allow 1 working day for
processing within the surgery, and
longer if posting is required. This
means if handed in at the surgery it
will be ready for collection after
2pm the following day.
If you request your prescription on
Friday this will not be ready for
collection until Monday after 2pm.
Please note if you drop your request
through the door over the weekend
this will not be picked up until
Monday morning and will not be ready
until Tuesday after 2pm.
Watch out for the messages on the
order form - they keep you informed
about your medications and issues
the surgery feel need to be brought
to your attention.
